No other alterations to the ad, copy or design are permitted. If you wish to make changes, you can submit your desired request to us at the contact information listed above. All proposed ads must allow 7-10 business days for company review. Additional time for state approval will vary.

All ads are good for six months from the date you receive your ad and can only be used in the states specified. Any use after the six month time period or in additional states must be re-approved by the home office. Requests for continued use of the ad or any other requests can be forwarded to the contact information listed above.
